Jesy Nelson celebrated the first birthday of her twin daughters, Ocean Jade and Story Monroe Nelson-Foster.
The singer's twins were diagnosed with Spinal Muscular Atrophy (SMA) Type 1 in January.
Nelson paid tribute to her "tiny superhuman" twins with a heartfelt video montage.
The video featured Nelson reciting the poem "Welcome to Holland" by Emily Perl Kingsley.
Nelson explained that the poem, which describes raising a child with a disability, beautifully summarises her daughters' journey.
