I think it had a military basis, as in "we'll 86 plan #2." But the only examples I can think of come from M.A.S.H and may be somewhat apocryphal.

It's one thing to tell the kitchen you're allergic to (say) shellfish and nuts and please be careful about cross contamination when preparing my chicken salad. It's quite another to basically ignore the menue and tell the cook to spontaneously create a dish that circumvents 20 or more common ingredients. Bonus points if the restaurant is already packed.
