That looks like a bottle cleaner/sanitizer. You put the bottle on the nipple like thing in the middle and when you push the bottle down, it shoots the liquid up into the bottle.
It's called a butter cream egg or butter egg. Outside of being very sweet, and for me, to be eaten by cutting off a piece, they are very tasty.
