The incident was attributed to a grave error by the shuttle bus driver, who mistakenly pressed the gas pedal instead of the brake, as reported by Honolulu police.
The driver had just finished dropping off passengers at Pier 2 when onlookers noticed the bus beginning to move forward. Reacting swiftly, the driver attempted to regain control of the vehicle but inadvertently accelerated, exacerbating the situation.
